A series of studies investigated the capacity of children between the ages of 7 and 12 to give free and informed consent to participation in psychological research. Children were reasonably accurate in describing the purpose of studies, but many did not understand the possible benefits or especially the possible risks of participating. In several studies children's consent was not affected by the knowledge that their parents had given their permission or by the parents saying that they would not be upset (...) if the children refused. In contrast, other studies found that children were much more likely to stop their participation if the experimenter said explicitly that she would not be upset if they stopped. We suggest that experimenters should pay more attention to describing the possible risks and benefits of participation in research, and that they should also make it clearer to children that they are free to stop once they have begun. The negative consequences of physicians' failure to establish and maintain personal relationships with patients are at the heart of the humanistic crisis in medicine. To resolve this crisis, a new model of doctor-patient interaction is proposed, based on the ideas of Martin Buber's philosophy of dialogue. This model shows how the physican may successfully combine the personal (I-Thou) and impersonal (I-It) aspects of medicine in three stages. These Three Stages of Medical Dialogue include:1. An Initial Personal Meeting stage, which initiates (...) the doctor-patient relationship and involves mutual confirmation; 2. An Examination stage, which requires a shift from a personal to an impersonal style of interaction; 3. An Integration Through Dialogue or Healing Through Meeting Stage, which involves the integration of the impersonal medical data into the ongoing dialogue between doctor and patient, as a basis for shared decision-making. The use of the model, as well as common failures of doctor-patient dialogue are discussed. (shrink)

À l’époque où la photographie n’existait pas encore, les reproductions d’oeuvres d’art et de peintures célèbres dominaient le marché de l’art. Au dix-huitième siècle, la comparaison entre les graveurs et les traducteurs était fort commune. Si, à l’époque, on accordait une grande importance à la fidélité des traductions aux textes originaux, de telles comparaisons amenaient à conclure à une sorte de submissivité purement technique des graveurs par rapport au talent génial des peintres. En examinant de près les copies réalisées, on (...) remarque toutefois que la fidélité à l’original n’était pas nécessairement la préoccupation première de ces traducteurs visuels. Ces derniers seraient plutôt des médiateurs. Cet article réexamine les actes de traduction visuelle du dix-huitième siècle afin de recadrer la pratique de la gravure pendant cette période et de permettre de mieux comprendre le mouvement qui irait d’un langage visuel artistique à un autre. Une étude de cas, celle de Nicolas de Launay, qui est sans doute l’un des plus grands graveurs du dix-huitième siècle, nous permettra de contextualiser cette pratique au coeur des débats historiques autour de la traduction. Notre objectif principal sera, finalement, de mettre en lumière les tensions qui existent entre les différentes disciplines associées à la peinture et à la gravure, ainsi que les processus complexes par le biais desquels les graveurs ont réussi à rester fidèles à la fois aux peintres, à leur public et à leur propre identité artistique. This study examined the quality of children's assent to a clinical trial. In subjects younger than 9 years of age, understanding of most aspects of the study was found to be poor to non-existent. Understanding of procedures was poor in almost all subjects. In addition, voluntariness may have been compromised in many subjects by their belief that failure to complete the study would displease others. If the fact that a child's assent has been obtained is used to justify the exposure (...) of that child to the potential harm of a non-therapeutic blood sample, the assent must be meaningful. In the nutrition study observed here, the quality of the assent of children younger than 9 years of age was very poor. The assent therefore did not provide a valid justification for requesting a blood sample from these children. This study indicates that most children younger than 9 years of age cannot be expected to consent or assent to clinical research in a meaningful way. The current age of 7 years for initiating assent (in addition to parental consent) is possibly not appropriate and should be reconsidered. (shrink)
